(Tampa, FL) – The National Committee for Quality Assurance (NCQA) has selected Healthmap Solutions, Inc. (Healthmap) for inclusion in its “Population Health Roadmap for Chronic Kidney Disease.” Healthmap was recognized for its innovative, holistic approach to kidney health management, which involves assessing patients’ social determinants of health, closing care gaps, and using proprietary algorithms to risk-stratify patients and recommend appropriate interventions.
“The incorporation of Healthmap’s best practices into the NCQA Roadmap represents further validation of our pioneering kidney health management program, with its focus on early CKD identification and intervention, timely nephrologist referral, advanced renal replacement planning, and integrated, patient-centric care management,” said Howard Shaps, MD, Chief Medical Officer at Healthmap.
The Roadmap is part of NCQA’s Kidney Health Toolkit. The Toolkit provides resources that enable patients, providers, health plans, and health systems to understand and promote best practices in kidney health and CKD care, especially among people with risk factors such as diabetes.
“Health plans and health systems can use the Roadmap to educate themselves on population management and understand how their processes align with NCQA program requirements,” said NCQA President Margaret E.
