(Tampa, FL) – Healthmap Solutions, Inc. (Healthmap), an industry-leading Kidney Population Health Management company, announced today that it has earned a three-year renewal of its Population Health Program (PHP) Accreditation from the National Committee for Quality Assurance (NCQA).

The announcement was made by Healthmap’s Chief Medical Officer, Howard Shaps, MD, who said, “Because its standards are developed with input from health plans, industry leaders, and other stakeholders and resources, NCQA accreditation represents an important validation of our innovative chronic kidney disease management program.” Dr. Shaps noted that Healthmap was one of the first kidney population health management companies to receive this important distinction.

The Healthmap program utilizes data-mining and predictive analytic technology to identify people living with kidney disease. This proprietary Artificial Intelligence (AI) technology is integrated with clinical expertise to deliver actionable, clinically proven insights and patient-centered care recommendations to healthcare providers. In addition to enabling collaboration among primary care providers and specialists, the program also connects directly with patients to provide the support and information they need to be fully engaged in their treatment plan. Since its inception, the Healthmap Kidney Health Management (KHM) program has been shown to achieve the value-based results that payers, health systems, accountable care organizations, and at-risk providers need and expect to deliver higher patient satisfaction and outcomes.

“The Population Health Management programs move us in greater alignment with the increased focus on person-centered population health management,” said NCQA President Margaret E. O’Kane. “Not only does it add value to existing quality improvement efforts; it also demonstrates an organization’s highest level of commitment to improving the quality of care that meets people’s needs.”

Eric Reimer, Healthmap’s Chief Executive Officer, said, “Our company is focused on the importance of quality in all that we do for our providers, patients, and clients. NCQA instantly provides credibility and value, and we are proud to partner with NCQA for our accreditation needs.”
