Healthmap Solutions (Healthmap) has announced the expansion of their industry-leading, NCQA-accredited Kidney Population Health Management program to fully eligible members in 27 additional states for one of Healthmap’s current clients. Healthmap’s partnership with this client initially began in July 2022, when the program was rolled out to the health plan’s eligible kidney members living in the states of North Carolina, South Carolina, Georgia, and Florida. As of September 2024, the program has expanded to serve the health plan’s eligible kidney members in 27 additional states. Healthmap manages this client’s kidney needs in a total of 31 states, and now manages the East Region (all states east of Texas) for the client.
