Mr. Chernin, Ms. Forte, and Dr. Subramanian Join Kidney Health Management Leader, Supporting Data-Driven, Population Health Management Approach to Clinical Care for Complex Kidney Patients

December 8, 2020, TAMPA, FL— Healthmap Solutions, Inc. (Healthmap), an industry-leading Kidney Health Management (KHM) company, today announced the appointment of Ethan Chernin as Vice President of Population Health Services, Helene Forte, RN, MS, as Vice President of Clinical Operations, and Mani Subramanian, M.D., MMI, as Vice President of Medical Informatics. These three recent appointments continue the expansion of Healthmap’s leadership team with top-tier talent, as Healthmap scales to meet the growing demand for its industry-leading KHM program.

As Vice President of Population Health, Ethan Chernin is responsible for advancing continuity of care and improving outcomes and costs associated with kidney care member populations. Prior to joining Healthmap, Chernin was a senior executive at BayCare Health System, serving as the organization’s Vice President of Population Health and Chief Operating Officer of BayCare Physician Partners. He was formerly the Executive Director of Rainbow & Babies Children’s Hospital and Senior Population Health Manager, Innovations, at the Cleveland Clinic. Chernin earned a BA in Political Science from the University of Michigan and an MBA, Health Systems Management from Case Western Reserve University Weatherhead School of Management.

Helene Forte will oversee clinical operations and provider relations. She joins Healthmap with 25 years of experience in population health and clinical operations, developing innovative strategies to expand provider and member engagement. Prior to joining Healthmap, she held several executive positions including Vice President of Dual Eligible Plans at BMC Healthnet, Vice President of Emerging Markets and Care Management for Tufts Health Plan, and Vice President of Medical Operations at APS Healthcare. She holds a BS in science with a concentration in nursing from the University of Massachusetts, and an MS in Nursing from Boston University.

As Vice President of Medical Informatics, Dr. Subramanian is responsible for leveraging data and informatics to improve patient outcomes and financial performance. He joins Healthmap with over 21 years of interdisciplinary experience spanning clinical medicine and informatics. He’s held several senior leadership roles overseeing clinical informatics applications, provider engagement, and technology integration. Prior to joining Healthmap, Dr. Subramanian served as the Chief Medical Informatics Officer – Ambulatory Services – with Presence Health in Illinois, leading the implementation of electronic health records (EHR) across a 12-hospital system. Previously, he served as the Lead Physician Informaticist at OSF HealthCare and as the Medical Informatics Fellow at the College of American Pathologists. He sits on several advisory boards of health technology start-up companies. Dr. Subramanian holds an MD from the PSG Institute of Medical Sciences/Research [India] and an MS in Medical Informatics from Northwestern University.

Mr. Chernin, Ms. Forte and Dr. Subramanian join Healthmap at a pivotal time in the Kidney Health Management sector, both across the industry and within the company. In the U.S. more than 37 million people live with chronic kidney disease and close to 800,000 have end-stage renal disease (ESRD), the more serious disease state which requires dialysis or a kidney transplant to survive. Kidney disease is complex and costly to treat. People living with the condition often present with comorbid conditions, such as diabetes and hypertension, requiring multidisciplinary care and an extensive medication regimen to remain healthy. In 2017, Medicare alone spent $120 billion to care for people living with kidney disease, up $6 billion from the prior year. Recent regulatory changes such as the 21st Century Cures Act are opening the door for thousands of newly eligible kidney patients to access more affordable care and a wider array of benefits through coverage under a Medicare Advantage plan.

These changes are driving up industry demand for solutions to improve outcomes, care, and costs.
