Although chronic kidney disease (CKD) is recognized as a national health crisis, only 3.4% of CKD patients are diagnosed in Stages 1–2. Most are not diagnosed until Stage 3 or later when kidney disease management becomes progressively more difficult. For better patient outcomes, kidney disease management must begin at the primary care level. That is the focus of our Issue Brief, Value-based Kidney Disease Management: It Starts Early.
