Chronic kidney disease (CKD) presents an unusual challenge for patients and the healthcare professionals who care for them. Patients are often asked to take action against a disease they cannot feel. CKD can progress without noticeable symptoms, even as kidney function declines. At the same time, many patients are managing other chronic conditions such as hypertension and diabetes, taking multiple medications, seeing several providers, and trying to understand changing recommendations about nutrition, physical activity, blood pressure, blood sugar, and other aspects of their health.

For health plans and providers seeking to slow CKD progression, identifying risks and recommending appropriate interventions are essential. But there is another important piece of the equation; helping patients understand what they can do, why it matters, and how to follow through.

###### **Turning Information Into Action**

Effective patient education goes beyond simply providing information. It extends to helping patients understand the connection between everyday decisions and their long-term kidney health.

This can begin with something as fundamental as understanding kidney function test results. Knowing what eGFR and urine albumin results represent, why changes over time matter, and why regular monitoring is important can make an otherwise largely invisible disease more tangible.

Education can also help patients understand why controlling blood pressure and blood sugar, taking medications as directed, following nutrition recommendations, avoiding potentially kidney-harming medications such as NSAIDs, and keeping recommended appointments can help protect kidney function.

This understanding is particularly important in chronic disease management, because many interventions are preventive. A medication or lifestyle change may not make a patient immediately feel different, but understanding its purpose can help provide the motivation to stay engaged with the care plan.

###### **Meeting Patients Where They Are**

There is no single approach to patient education that works for everyone. Patients with CKD may be managing multiple complex comorbidities and receiving care from PCPs, nephrologists, cardiologists, endocrinologists, and other providers. Fragmented care can result in inconsistent or incomplete information. Social barriers may limit access to technology or educational resources, while language differences and limited health literacy can make complicated medical information even more difficult to understand.

For education to be effective, it must be clear, accessible, culturally responsive, and reinforced over time. The [National CLAS Standards](https://thinkculturalhealth.hhs.gov/assets/pdfs/EnhancedNationalCLASStandards_Revised_June2025.pdf)2 provide a framework for eliminating healthcare disparities when delivering patient education, which is critical for any healthcare organization. Healthmap Solutions applies these standards when developing patient education strategies and materials to help support culturally and linguistically appropriate communication.

When speaking to patients or developing written educational content, it is important to be mindful of patients’ literacy levels when explaining complex medical information. To support understanding, Healthmap typically develops member-facing materials at a fourth to sixth grade reading level. Education should also reflect where an individual patient is in the kidney disease journey. Someone with earlier-stage CKD and uncontrolled hypertension may need help understanding blood pressure targets, medications, nutrition, and the importance of continued monitoring. A patient with later-stage CKD may need education about dialysis options, transplant evaluation, and/or conservative care.

The objective is not to overwhelm patients with everything they might eventually need to know. It is to provide relevant information that helps them take the *next best steps* based on their current needs, risks, and personal goals.

That individualized approach can also include family members or other care partners when the patient chooses, extending education and support to the people who may be helping the patient manage care at home.

###### **Reinforcing Education Between Provider Visits**

Healthcare providers have an important role in ensuring patients understand their condition and treatment plan. However, the complexity of CKD and its common comorbidities can require education and reinforcement that extend beyond the clinical encounter.
